what information is needed to make a reservation?

Dear all, I am planing to have a trip to Beijing and Xian for one weeks. This is my first time to book online for a tour to China. I have no idea about what information is needed to make a reservaton. Could anyone help me?

1.What is your country of residency?

2. How many are traveling together with you (including yourself)?

3.Could you please let us know all the names, ages and passport numbers?

Please write your names (as it appears in your passport) clearly in the below format.

Mr. or Mrs. Your name; Birthday, Gender, Passport number; Nationality,Adult or child (less than 12 years old)

4.How many days do you intend to stay in China?

5.What are the places you 'must' go?

6.What are the things you 'must' do?

7.If there are children traveling with you, please tell us their ages. There are significances price differnecesfor children 'under 2 years old', 'under 12 years old' and 'above 12 years old'. Are they above 1 meter/3 ft 3 inches tall or under 1 meter/3 ft 3 inches

8.What's your date of arrival in China and departure date from China?

9.What is your arrival city and departure city in China?

10.Do you have your international flight arranged to China already? if so, could you please let us know your flight number and time, arriving city?

11.Do you have your international flight leaving China already? if so, could you please let us know your flight number and time, departure city?

12.What kind of hotels do you prefer, 3,4,or 5 star hotels? How many rooms would like to have? Do you need extra bed?

13.Can you give us an idea of your budget per person?

14.Do you have any other special requirement?

15.Could you please let us know your other ways of contacts other than your E-mail address, such as your telephone number or your fax number?
